Your Application fee of $25 is due now; it will be paid on the the next page. Upon receipt of your application, you will log into your account to submit your Specimen and receive your clearance letter to sit for your National Exam. If you would like to be Board Certified in more than one Educational track, you will need to fill out this form and application again for your additional Board Certification. Your exam fee will be paid at your testing center.
